Uninsured Services

Please be aware that some services offered at Etobicoke Central Clinic are not covered by OHIP. These uninsured services require payment by the patient. The following is a list of some examples of such services:
Completion of Forms for Physicals:
● School/camp physicals
● Truck driver’s physical

● Sick notes, back-to-work notes, daycare notes
● Attending physician statements
● Admission to daycare, preschool, community college, university, or institutes
● Disability tax credit forms
● Immigration letters/invitations
● CRA letters
● Insurance-related sick notes/forms

Travel-Related Services:
Travel cancellation insurance forms/certificates
Travel immunizations (per injection, not covered by OHIP). Please note that we do not offer travel counseling at our clinic. For comprehensive travel advice and recommendations, we highly recommend visiting a specialized travel clinic.

Other Services NOT Covered by OHIP:

Ear irrigation
Wart removal (hands)
Skin tag removal
Regular office visits (for non-OHIP patients)
Missed appointments
TB Skin Test and reading for non-OHIP visits
Photocopying of charts or chart transfers

Please note that the TB test for educational purposes requires proof and is covered by OHIP. If you have any questions or concerns about these uninsured services or their associated costs, please do not hesitate to contact our office for more information.

OHIP and Non-OHIP Visits: We’ve Got You Covered
For students requiring a TB skin test, the cost is often covered by OHIP. Just bring your health card to your appointment. For employment or other purposes, patients are responsible for the service fee. Regardless of the reason for testing, Etobicoke Central Clinic is dedicated to providing accessible services without compromising quality.

Emergency Situations
If you experience a life-threatening emergency, such as chest pain, severe bleeding, or symptoms suggestive of a stroke, please call 911 or visit the nearest emergency room.
